A 74 year old man has a routine chest X-ray (click on the image to see a larger version) during a pre-operative assessment for a hernia repair. Following this, he is referred to the respiratory clinic for assessment. He is asymptomatic with no chest symptoms. He is an ex-smoker with a 10 pack year history. His SpO2 is 96% on air. His chest is clear to auscultation and examination is otherwise unremarkable. His FEV1 is 2.3 (86% predicted) and VC 3.7 (84% predicted).
